React Native in Production: Scaling Cross-Platform Healthcare Apps to 100K+ Users

Shipping healthcare products with over 100,000+ downloads across iOS and Android taught me that mobile engineering requires deep discipline in state hydration, native gesture threads, and offline resilience.

1. Mobile & Web Convergence with TypeScript

By organizing our codebase into shared TypeScript packages, data models, Zod validation schemas, and utility functions are shared 1:1 between our Next.js web portals and React Native mobile applications.

2. Preventing Navigation Deadlocks & Race Conditions

In complex multi-step patient onboarding flows, asynchronous push notification deep-links can attempt to mount screens before authentication state resolves. We solved this by implementing strict navigation guards backed by state machine checks.

3. Offline-First Data Sync for Health Metrics

Patients frequently record blood glucose and meal logs in areas with poor cellular reception. Logs are stored in local SQLite/WatermelonDB storage and synced to PostgreSQL via idempotent upsert endpoints upon network reconnection.

4. Native Gestures & 60fps Mobile Animations

Using react-native-reanimated and react-native-gesture-handler, all gesture calculations run directly on the UI thread, ensuring zero frame drops during interactive sheet dragging and data scrubbers.
